No, if you know the total amount of eligible medical expenses, you do not need to break them down for entry on the separate lines in TurboTax.  The separate lines in TurboTax are to assist you with entering everything you are entitled to, and not duplicate any expenses.

You put medical expenses such as ambulance and medical transport costs in the Federal>>Deductions and Credits>Medical>Medical Expenses section.  Or you can click the "magnifying glass Search" icon, enter "medical expenses" in the search window and hit return, and then click on the "Jump to medical expenses" link.

Note that in order for these expenses to reduce your taxes, you must Itemize Deductions, and your total medical expenses must exceed 7.5% of your adjusted gross income (AGI).  When you enter your medical expenses and other itemized deductions (typically taxes, mortgage interest, charitable contributions) TurboTax will make the calculations and choose the options that result in the lowest tax.
