I am lost as to properly fill out this section. I belong to a health care provider...Kaiser Permanente...and pay monthly fees. I also pay my share of medical expenses under Medicare. I have no idea under what category I enter my premiums.

Enter them all under Medical Insurance premiums. The IRS gets the total amount of all your medical, dental and vision expenses and doesn't see the categories. The breakdown is for your benefit so you can keep track of what you entered.
