My total medical mileage from 2019 is 7890. @.20 per miles =$1578. The turbotax deduction im seeing is $362. Why is there a difference?

It sounds like you have entered medical miles driven based on the .20 rate.  Your medical expense deduction is limited to the amount that exceeds 7.5% of your Adjusted Gross Income (AGI).  However, I can't be 100% sure from your question where you are seeing the number and to exactly what it applies.
