My wife is a Realtor and received a W-2 in 2017 (vs. a 1099). Where can we deduct all of her expenses (mileage, fees, meals, gifts) ? (Schedule C, Schedule A, etc.)?

Not on Schedule C, since she is not self-employed.  See this re Schedule A:

And be aware that, starting with the 2018 tax year (returns filed in early 2019), unreimbursed employee business expenses are no longer deductible.

How does she collect her commissions ?

They were paid separately  from her salary (once a month in a commission check) and will be reported on her W-2 for 2017.
