Repayment of Sign-On Bonus within the same year - how to file for tax refund?

I started a new job in 2022 and received a $5,000 sign-on bonus, but resigned within the same year and also repaid the full bonus (including taxes) within the same year (2022). How do I file for a tax return on the bonus repayment? I entered the W2 info from that job, but my tax return has not changed after entering the info. Did the employer provide the incorrect dollar amount on the W2 or do I need to file an addendum in order to receive my tax refund? Any insight on this matter is greatly appreciated!

    The W-2 should include the effect of the adjustment, if the wages were repaid in the same year.  There is no procedure on your tax return to claim an offset for a bonus that was repaid in the same year.  The company should adjust your box 1, 3 and 5 wages and your box 4 and 6 taxes.  If you don't know if your W-2 correctly reflects the repayment, you may need to check with the employer, or maybe take your pay stubs and other information to an accountant to review.
