Resident Alien with 1042-S (income code 20, exemption code 04, 16) and W2

Apologies on posting a somewhat redundant question. I've seen some conflicting info and I have 2 exemption codes, hence this post.
I am currently a resident alien on F1. I received two W2s. I also received an 1042-S from my school with an Income code 20, exemption code (in box 3a) 04 and exemption code (in box 4a) 16. I don't have 1040 or 1098-T. From what I see in the forum, I need to add the amount in 1042-S at Misc income under federal and cannot e-file. I am also eligible for a tax treaty exemption (I guess hence the exemption code 04).
I wonder how I should file my tax. If I put the amount in 1042-S in the Misc income under federal and mail the return like other answers suggest, do I still attach the copies of 1042-S to my return (like the header in 1042-S suggest)? If I directly attach the 1042-S to my state and federal return and mail it, do I still need to add the number to the Misc income? I also wonder if I can give up claiming the tax treaty exemption so that I can e-file (without filing 8833). My final question would be if I need to file anything related to my 1042-S in my state return.
