Are you referring to premiums for these things that your employer is paying for you (often collectively called "Section 125 benefits")?

Did your employer remove these amounts from Wages in boxes 1, 3, and 5 on your W-2 (you will have to ask them - you can't tell from the form)?

If yes, then enter these items as printed on your W-2, and chose the category of "other" for them.

If no, ask your employer why they did not include these items in box 12 with a code of DD?
