Significantly more was contributed to my HSA during 2021 than what is listed in box 12 with code W- should I request a corrected W2?

Only what I contributed is listed, not what my employer contributed ($1,083.47 more than my contribution).

No, you do not need to request a corrected W-2.  

 

As you go through the Health Savings Account (HSA) section of your return, watch for the screen titled 'Did your employer tell you about any other contributions?

 

Answer 'yes' and some fields will open where you can enter 'Employer and payroll contributions not reported in Box 12 of your W-2'.  This is where you will enter the contributions to the HSA that were made by your employer.

 

This screen should pop up just after the one where you could enter your own personal contributions outside of payroll deductions.

if your employer contributed to your HSA that should be included in code W.  request a corrected w-2.

you should have also gotten a form 5498-SA for the total contributions made.

Employer contributions (including contributions through a cafeteria plan) include any amount an employer
contributes to any HSA for you for 2021. These contributions should be shown in box 12 of Form W-2 with code W.
