There is a section in Turbo Tax under HSA which asks: Did your employer tell you about any other contributions?

I answered YES as my employer made contribution and I made contribution as well.
For 2021 W2 - Box 12 - W has $3600 in it. My employer contributed $1350 and I contributed $2250 through my employer payroll.
What shall I put in question on
- "Employer and payroll contributions made in 2021 for tax year 2020?"
- "Employer and payroll contributions made in 2022 for tax year 2021?"

Your employer did not tell you about any other contributions. The only contributions to your HSA were the amount in W-2 box 12 with code W. You just enter that when you enter your W-2 information. TurboTax will pick that up automatically. You don't enter any other HSA contributions anywhere else.

Go back to the screen that asked if your employer told you about any other contributions. If you entered anything in any of the three "Employer and payroll contributions . . ." boxes, delete what you entered. Leave all three boxes empty. Then change your answer to No and continue through the rest of the questions.

On Form 8889 the full amount from box 12 of your W-2 is called employer contributions, even though part of it came out of your pay.
