Question

W-2 box 12c shows $3499.99 but my HSA account shows no such contribution. Is there an error on my W-2?

I have 2 W-2 from 2 jobs. The other W-2 has $800 in box 12c which is correct.

It may seem silly but does your HSA account show a contribution for $3,500? Lots of people round numbers to avoid dealing with decimals.

 

If the code W amount is $3,500 on one W-2, but the HSA custodian says that it was not sent by your employer, then you need to contact your employer and ask them where they sent that money.

 

At this point, you may need to extend your return while you get this straightened out.
