We had + $5K in medical exp in '17. for supplemental ins. , dr visits & RX. I am retired & spouse is pt time. Have not itemize. Do we claim our med exp for '18?

If you only have $5,000 in medical expenses in 2018 again, then no you would not claim your medical expenses as the Standard Deduction has increased to $24,000 ($26,600) if you are both over 65.  So your standard deduction would be higher than your itemized deduction, therefore you would do better to take the standard deduction.
