Solved

What dollar amount does it want when asking what amount do I personally contribute towards my HSA? Box 12c, 12d or 12dd? The question just states from box 12.

When you entered your W-2, the amount with code W (no matter which box 12 it was in), is automatically caried to the HSA interview. You do NOT enter this amount yourself in the HSA interview.

NOTE: the code W amount if called the "employer's contribution" without regard as to whether the employer contributed it or you did by payroll deduction. Do NOT enter any part of the code W amount in the HSA interview as a "personal" contribution. A personal contribution is when you send contributions directly to the HSA, not through your employer.

This is the terminology that the IRS uses, and we have to follow it.
