What is the difference between medical insurance premiums and medical professional fees?

I have incurred out of pocked expenses for medical professional services that my insurance will not cover. Do I add them to the Medical Insurance Premium section or Medical Professional Fees section?

Well it's not Insurance but out of pocket expenses.  Actually it doesn't matter where you enter it.  Only the grand total goes to Schedule A.
