Question

When I had surgery I was moved to another hospital by ambulance and I paid by mile. Does the amount I paid goes under medical miles driven ?

No, do not enter it under the medical miles section.

Medical miles driven is for use of your personal vehicle.  Instead, the cost that you paid to be transported by ambulance should be entered in the Other (Bus, Parking, Taxi, etc.) box on that same page.
