Where do I donate to a 501(c)(3) plan on my Arizona tax return?

Form 321 is used to claim the donation. It's listed under the credits section of your state tax return. See attachment.

I enter the code for Sunshine Acres is 20037 on the state form and the link from the section.  When I enter the number I get a message the QCO doesn't match please enter correct number.  I am stuck and can't go further.
