Where do I put a form from bank that is "Total Interest Paid" but has no form

    Sounds like the bank is reporting the total interest paid to you.  That entry would be for a form 1099-INT.  In the "Income" tab, use the "Interest and Dividends" topic.  Enter the bank name and the interest amount in box 1.
