How do I delete the College Tuition Qualified Expenses Optimization Worksheet? I started to fill it out, and do not qualify, but an now asked for the College EIN.

I was asked the questions on this:

College Tuition Qual Exp Optimization Wk: Undergraduate Yes/No must be entered

- No Entry

- Undergraduate Yes/Box

- Undergraduate No/Box

I did not attend school in New York and do not qualify for this either. It is taking information from my New Jersey school. The tuition expense is wrong and it is telling me I owe $34 to NY because of this form. How do I fix this? How do I get rid of this form or re-fillout the form?
