Are you referring to a 1098-T as your college tax form? If you are entering this is the correct place; you are not asked about a box d.

 

Ensure you are in the Deductions and Credits area under Education. Be sure to delete any entries you already made if you were entering this information in the wrong place.

You need to be more specific. Nobody here knows anything about a "college tax form" with a box d.  For most  students (or parents of students) the only form that matters is the 1098-T, issued by the college showing tuition paid and scholarships received.
