The1099-Q does not contain any entry for the state. I believe the plan is based in California even though I have never lived there. The amount of distribution was made to pay for dual enrollment college classes for my son. What state do I put for this? California or Florida (where I live and my son goes to school for dual enrollment - high school and college).
