Did you apply for and purchase health care insurance through one of the state Marketplace Exchanges?  If so, you should have received a Form 1095-A.  The form is required to be reported on your federal tax return.

If you did not have Marketplace Insurance then the IRS has incorrect information.  Contact the IRS using the phone number on the notice you received to resolve this matter.
