Question

1099-DIV says "leave it blank if it doesn't apply to you" but the form is forcing me to input information that doesn't exist.

There is no box 13, 14, or 15 on my 1099-DIV

Go back to the 1099-DIV form and edit it. remove any information such as the two-letter state or $0 in those boxes.

Leave the FATCA box unchecked.
