1099-R Box 15

Box 14 of 1099-R has my State withheld tax but box 15 just says MD and not the State payer's number. This number was always included in the past.

    I have one 1099-R provider, who used to leave that state number box empty, and I had to call them to get that number......and yes, I had withholding for the state in box 14.

    After two years of calling, I started just using the same number that they told me to use the prior years (hadn't changed).

    Luckily, they've started including that state number on their 1099-R forms recently.

    ...but whoever is providing your 1099-R hasn't started doing that (yet).  So call them on the phone and ask.

    I found additional info on an IRS sight that said only 10 States require the State # on the 1099-R form..  Maryland was not listed among the 10 that require it. However, they did suggest contacting the producer of the 1099-R to get them to furnish it at some future date.  In fact they said the Federal EIN number on all 1099-R forms is sufficient to locate the Maryland # or any State # if needed by the government.
