After entering 1099-R information there is a page "Let's Verify This Entry". All the items in parentheses are incorrect. The State is not Box 15 but Box 13. The Sate ID is not Box 15 but Box 13. The State Distribution is not Box 16 but Box14. The Amount Withheld is not Box 14 but Box 12.
