2 sets of boxes 18, 19, 20 for the same locality

On my W-2, I have 2 sets of boxes 18, 19, 20. Both 20 boxes state the same locality, so I get an error message when I try to enter them on separate lines. I saw other posts that suggested combining the amounts.

 

The amounts in the two boxes labeled 18 are different. One of them matches the income listed in box 1 and the other matches the income listed in box 5. So, combining them would not be accurate - it would nearly double my earned income. Which should I choose to list in box 18? 

 

The amounts listed in the two boxes labeled 19 also do not match. Per my paycheck, both amounts were withheld  - I'm guessing one is city and one is school district.  Should I just add those together?  Thanks for any assistance.

    I assume you're in Ohio.

    That's probably right if you live in a School District (SD) with an income tax and either live or work in a city with an income tax. 

    Box 20 should have told you that, but you say " Both 20 boxes state the same locality".  It sounds like your employer made a mistake.  Ideally, you get a corrected W-2, from your employer. Lacking that, change the box 20 (on the line with the box 1 amount in box 18) to the SD number that you know the tax was withheld for.  Typically, Ohio cities tax the box 5 amount and SD's only tax the box 1 amount.
