2021 Oklahoma form 512-Form 506 Investment/New Jobs Credit

I have been trying to get help on this for months now. I cannot file my 2021 Oklahoma 512 form because it is wanting me to fill out form 506 with at least $50.000.00 on it. Two things; Nothing on this form pertains to my business, nor do I have any employees. It wants me to fill out for 2018 and 2019. I have gone through the help desk for software problems, and there are none. I also talked for over 30 minutes with a CPA/Tax Expert/Tax Advisor. She was supposed to get back with me the next day. It has been 3 business days and I still have not heard back from her. Any ideas or suggestions?
