2022 tax return Personal income 2021 tax questions

I'm trying to answer the following questions:

1. Did I make my 4th quarter estimated tax payment for my 2021 (STATE) taxes after dec 31st, 2021? 

2. Did I send in a payment with my 2021 (State) tax extension request?

3. Did I make a payment when I filed my 2021 (State) tax return?

 

4. Did I apple some or all of my 2021 (State) refund toward 2022 estimated tax?

I have a pdf that I can view for my 2021 tax return. Where would I look on the form to answer these questions?

only No.4 can be found on your tax return, at the bottom.

you have to look at your checkbook for the rest.

 

payments to state are itemized on your 2021 return if paid in 2021but on your 2022 return if paid in 2022
