401K and 401A

In 2021, I maxed out my 401K contribution and my employer put remaining funds into a 401A.  I moved the funds in the 401A over to a ROTH IRA.  I didn't take any distribution of funds.  I never received a 1099R form.  Because the funds that were put into the 401A were taxed, do I need to show anything on my return for moving those funds to a ROTH IRA?

    Contact the plan administrator to obtain the missing Form 1099-R.  They are required to provide one.
