401K withdrawal

I resigned from my job and I withdraw all my 401k last year. I can't finish my filing because I don't have this form 1096 or 1099-R. Only the screenshots from my employer when I did the withdrawal. The total amount and my Federal tax deduction and State tax. I like to add this amount to my income tax return this year. My question, how can I add my 401k withdrawal for my online Turbo Tax filing?

    The 401(k) administrator was required to send you a Form 1099-R, postmarked no later than February 1.  You should contact them to get a copy of that form.  I would not file a tax return until you receive that form so you can properly report the distribution.
