Box 12d of my W-2, code W notes $2,425.00 to HSA. $1,000 was by my employer and the remaining $1,425 was contributed from my paycheck. Do I need to note that differently?

No need to make notations.  The W code notes what was submitted to your HSA administrator through payroll.  It doesn't matter who contributed the dollars since your annual limit is the total combined contributions, regardless of source.
