Colonial Life says they are not issuing a 1099G for my Short Term Disability? The funds were issued straight from them, and not my employer. How do I file it or do I not?

    Since you paid for the disability insurance then the payments received are not income to be reported on your tax return.

    Me, through my paycheck. It is deducted by my employer when they pay me each month.
