Confirm Address on 1099-G

I lived in California for 11 months of 2020 before ultimately moving to Utah.  Benefits were received at my home address in CA before I sold it in November 2020.  

 

Turbotax is asking that I confirm the address on my 1099-G from California FTB.  On the form itself there is no address listed.  On the USPS envelope the form came in, it has my temporary California address (parents) I used for mail before I made the final move to Utah with my new permanent address.  I did notify California of each address change.  What address do I use for this question, "Let's confirm the address listed on you 1099-G?

 

Should I use the one printed on the outside of the envelope, my original address where I received the benefits, or my new current permanent address in Utah?

You would enter the address on the envelope, as that is the address the state has for you.
