DCFSA left over amount reporting

I contributed to it as part of my first employer where I worked for little over a month, I chose to contribute $5000, $625 was deducted from my pay for the time I worked on. I haven't used any of it. How do I report it for the amount left over question? The DCFSA account shows $5000 as remaining, the W2 shows $625 as the money I contributed?

It appears that what you contributed may have been after tax while what was reported on the W2 amount was pre-tax. You should check with your employer to verify. If that is the case the remaining amount would be the $625. If you contributed pre-tax then the $5000 is remaining.
