Do I answer "no" to question 1b under Part 1' Qualifying Information (Has the Franchise Tax Board (FTB) previously disallowed your California EITC?) ?

It is asking you if the Franchise Tax Board (FTB), (California's version of a department of revenue, has ever told you (they would have sent a letter) that you were not going to get the California Earned Income Credit. You would had to have included the EITC on your California Tax Return. If you are unsure, the answer is probably no, but only you and the FTB know whether you should answer yes or no.
