Do I file the w-2 city or local info on FED or STATE w-2 ? or is there a separate form... I didn't see an option .... thanks in advance...

Depending on what locality or city tax you are filing, it might be included in the state return. If it's not included, you would have to file it separately by hand, or with another preparer.
