How do I generate for 14039?

Form 14039  is the IdentityTheft Affidavit. You may have inadvertently indicated that you were a victim of Identity Theft. The form will populate automatically in this instance as a security measure. If you were not a victim of Identity Theft you do not need the form. Please follow the steps below to delete the form in TurboTax Online.

  • Sign into your Online account.
  • Go to My Account at the top then Tools.
  • In the Tools Center pop up window select Delete a Form.
  • Then select Delete next to Form 14039 and confirm the deletion.

There seems to be a problem with the TurboTax online instructions regarding the need to fill out IRS form 14039 (Identity Theft Affidavit).  The instruction states:  if you've EVER been a victim – or suspect you've been a victim of identify theft – you need to fill out this form.  According to an IRS phone conversation on April 25, 2018 this form applies to the current tax year.
