Do I have to fill out the print off and mail it myself when I file by mail or does it send my info I already submitted and mails my check back?

if you are not e-filing it's up to you to mail the return. follow Turbotax filing instructions. in addition, I recommend a method whereby you can either track the return or get proof of delivery. the USPS is having its own issues with getting mail delivered.  

If you elect the option to file by mail, then you have to print, sign and mail the return to the IRS. When you select that option and instruction sheet will also be printed. That instruction sheet informs you have what documents you need to include with your mailed return, along with the address to mail it to.

If a state return is involved here, then a physically separate instruction sheet for the state tax return is printed. The mailing address for the state will not be the same as it is for the federal return. So pay attention to details.
