The forms in question are US forms and printed with the state forms for submission. Just want confirmation that they are required for state tax returns.
From the MA instructions ... since the program printed those federal forms I would believe them to be part of the "other applicable schedules" ... be safe and send them it can't hurt.
Now that you have completed Form 1, sign your
name at the bottom of page 1 of Form 1. Your
spouse must also sign if this is a joint return. Write
the date you signed the return.
Note: Be sure to include all four pages of Form 1,
as well as Schedule HC and all other applicable
schedules.
Enclose with your Form 1 all state copies of your
Forms W-2, W-2G, PWH-WA, 2G and any Forms
1099 which included Massachusetts withholding.
