My employer over withheld Social Security and Medicare tax on my W-2.  My employer doesn't want to reimburse me.  Can I get the excess back when I file my 1040?  If not, how can I get reimbursed?

No, you can't get the money back when you file your tax return. You will have to file Form 843 separately from your tax return to get the refund if your employer won't do it.

See this TurboTax support FAQ for reimbursing SS taxes withheld - https://ttlc.intuit.com/turbotax-support/en-us/help-article/social-security-taxes/get-refund-excess-social-security-tax-withheld/L1JTKMb7Q_US_en_US?uid=m7jl7y93

Look at your W-2 for the Medicare taxes withheld.  Multiply the amount in box 5 by 1.45%, that should be the amount in box 6.
