I’ve always files jointly for them.

But in January my father was approved for Medicaid for Long Term Care in a nursing home.  Does it change how I need to file?

Both get annuity payments from the government and live in Ohio

Do I file married jointly or separately?

Are they considered in the same household now?

Living together under the same roof is not a requirement to qualify to file joint. Only being "legally married" on or before Dec 31 of the tax year is required. You'll still file joint. So in a nutshell, nothing changes. (Unless you have additional information to provide)

Besides, for a married couple to file separately is the absolute worst and most costly way for them to file tax-wise. For one thing, that makes significantly more of their SS income taxable.
