Form 3115 vs Form 8453 (different Mailing address)

I needed to file Form 3115, which triggered Form 8453. I have successfully e-filed to the Federal, with a copy of Form 3115 and 8453. According to the instruction, I need to mail a copy of these forms. 

 

1. Form 8453 - indicated that I should mail it to IRS at Austin , TX

2. Form 3115 - indicated that I should mail it to IRS at Ogden, UT 

 

My question, specifically, is what exactly to include and to which address - should I mail/include a copy of Form 1040, Form 8453, Form 3115 and the attachments, to EACH of the 2 addresses above?

You are probably better off faxing the form to the IRS (otherwise mail it to the address applicable to the 3115).

I had planned to do that (mail to the UT address according to the IRS website).

 

But then I saw the address printed on FORM 8453, which has a different mailing address (in TX). So wondering if I should send 2 separate mails and with what content?

You could send the forms and statement to both addresses. However, it would be infinitely easier to fax them to the IRS fax number.
