Got a 1099-R but do not have retirement plan

so I just got a 1099-R form in the mail today. I never had a life insurance/retirement plan with my old job (just got a new on beginning of January) so I'm super confused as to why I got this. luckily the amount distributed/taxed on there does not appear to be a lot, but I'm still worried because there is sensitive info on the form and I don't know where it came from. anything helps, thank you

    You should be somewhat concerned. I would try to find out where it came from through your former employer and if they don't have an answer, report it to the IRS using the link below. Don't give out your Social Security Number to someone you don't know.
