If you are receiving distributions from your 401k, then you should have received a Form 1099-R to use for your tax return. If you did not get Form 1099-R, but you received a distribution during 2020, then you should contact your 401k plan administrator to get a replacement copy. If you have online account access, your Form 1099-R may be available to you online.

The Form 1099-R reports a lot of information about your retirement plan distribution that will be required in order to be sure that the correct amount is being taxed on your return.
