Hello, do I need to include my 1040 form with my payment? The payment voucher was the only document to pop up. Thank you!

No, the Form 1040 is not sent with your payment. You only need the payment voucher, Form 1040-V, to be included with your check to the IRS for the federal taxes owed.

If you e-filed your tax return and it wax accepted, you do not need to mail in the Form 1040. When you mail your check or money order just put the 1040V voucher in the envelope with your payment. Write your SSN on your check. I suggest using certified mail when you mail anything to the IRS so that you will know it was received.
