Box 6 of Form W-2 is 1.45% of your Box 5 amount.

Additional Medicare tax is the additional 0.9% Medicare tax on the portion of wages, self-employment, and railroad retirement income that exceeds a certain threshold. This tax is reported on Form 8959.

The threshold for additional Medicare tax is $250,000 for MFJ, $125,000 for MFS, and $200,000 for other filing status.
