How do I file a sign on bonus that I received from my employer?

Your sign on bonus should be included in with your W-2 income as it is treated like regular wages.  If so, you will just enter the W-2 as normal and the income will be taxed as your regular income would be.

 

If for some reason, it was not included in your wages, you will need to contact your employer to get a corrected W-2 as a bonus is subject to FICA taxes just like your regular wages are.
