Do you mean form 4684?

 

Form 4684 is for reporting casualty and theft losses on your federal tax return. If you are seeing that form referenced and you don't have a casualty or theft loss, you can delete the form as follows:

 

1. Find the Tax Tools option on your left menu bar and click on it

2. Click on Tools

3. Find Delete a Form and click on it

4. Look for form 4684 in the list of forms and click on the option to delete it

**Say "Thanks" by clicking the thumb icon in a post**Mark the post that answers your question by clicking on "Mark as Best Answer"
