How do I handle income where the employer gives me a sticky note with a number on it and says here is your 1099 from the company?

I no longer work for this employer, they gave me a normal W2 for working on one business of theirs and a hand written sticky note for the money earned from the other.

If the note your employer gave to you doesn't look like Form 1099, he might didn't file it properly. However, you still need to report the money received as income. You can find amount of your income on your final pay stub. If it is not available, use your best knowledge to figure the amount of your income from the employer for reporting purposes.
