How do I report a voided 1095-A form?

I had an insurance broker take my identity and fraudulently sign me up for health insurance marketplace, so the IRS rejected my taxes. I wasn't made aware this existed until my taxes were rejected. I did investigation and found how it was fraudulently filed and requested help from the health insurance marketplace. They mailed me a voided 1095-A form and advised that I'd just need to produce the form with filing my taxes. I'm not sure where to produce this. When I go to fill out the form information, there's no option to submit a voided form. How do I submit this and fix it and refile?

If the 1095-A was voided, you should be able to disregard the marketplace health insurance that was fraudulently started in your name and not have to enter any information regarding Form 1095-A or the voided Form 1095-A. Per the IRS, the voided Form 1095-A – as the well as the previously received Form 1095-A – should not be used to file your tax return. In this case, you would keep a copy of the voided 1095-A on file for your records only.

If your return continues to be rejected, then you must print and mail in your return. For instructions to print by mail, please click here

Corrected, incorrect, or voided Form 1095-A
