How do I report income from Option Cancellation from my former employer getting acquired?

I have not received a w2 or 1099, and I have no contacts at the new company, which is so large that I would not even know where to begin to get through their phone tree for information.

An Option Cancellation should be reported to you on a W-2 as compensation from your old company.  Did you receive a 1099-B for a Stock Sale? 

 

You may have to negotiate the phone tree of the new company, unfortunately; maybe start with Human Resources.
