I received short term disability benefits that were not reported on my W2 from my employer. My employer pays the premiums and the payments came from the third party insurer. I have an EOB for all the payments but no W2 from the insurer. Should I contact the insurer for the W2? I tried to enter the income as additional but there was no way to account for the taxes already withheld.
